Commission application program codes and designator codes?

I've been looking all over the internet for these. The ENS that I am going through for my package is not really setting me up for success. Sent me the form and said "fill it out". It's a "pull" situation. Rather than sit and wait for someone in the regional naval recruiting office to get back to me; Does anyone know where I can find these? Looking for NFO, Aviation Maint. Duty Officer and Intelligence Officer.

Your recruiter is supposed to fill these out for you based on your designator choices once you return the "nearly" completed form back to them.
