Just fess up. Tell them you didn’t really think about it when filling out the forms because it has been so long. No. I'm saying PRK with no complications does not require a waiver. In an examiner's chair the tissue that grew back after the procedure is clearly different from the tissue around it. You cannot hide PRK from an optometrist.so are you saying they didnt detect it when you went through NAMI?
Did you have to provide any pre-op/post-op supporting documents? Since it was so long ago, I would imagine it hard to retrieveI had to disclose a bunch of stuff up front on accession, commissioning, and again for NAMI and NASIS. Including PRK. I didn’t end up requiring a single waiver and they gave me a clearance without issue.
The best time to have said something was initially, the next best time would be now.
