I can't get a hold of my recruiter so I'm hoping somebody here can help me out. I am having a former employer fill out a request for reference DD370 form. We were just wondering what goes in the "To" box right above where it says: "Applicant Identification Data". And also if the form needs to be typed or handwritten.

In the "to" box put your recruiters address at the NRD. I don't think it makes a difference if it is typed or handwritten, just make sure if it is typed that the person still signs it.

When I was having my references sent in I found it was best just to have the person filling it out fax it in because a couple letters seemed to get lost in the mix at the recruiters office.
