Requesting LORs

I have been working on my application since this winter (need to wait until the summer to apply since I'll be 6 months from graduation) and I am at the point where I feel I should be asking former employers, professors, etc. for LORs. Should I be printing out the DD 370 form or email it to my recommenders or is that up to my OR since there is info he needs to fill out on the form? I'm curious because I have one recommender moving out of state soon and a couple who I do not have e-mail addresses for. Thanks for any info/advice.

Get them to fill out a DD 370, then give to your recruiter.

+1. They can either mail or scan/email the form to us. Personally, I prefer the latter.
