Statement form USMC

Does anyone know the USMC (or if they use another Department) form number for the army equivalent of a sworn statement or some other "official" additional comments form that is generic and in wide use? Thanks in advance, I didn't have much luck in google searching and I was not sure if the DoN versions would suffice.

You do realize that the USMC is part of the DoN, right??
