I was told the same thing. It has to be issued by the state and not the hospital. Most states have a request form on their websites some place.

Recruiters CANNOT use the one issued by the hospital. You can, for most states, order the State's birth certificate online.

For your state (Allow 4-6 weeks for processing applications):

While looking through the NAVYOCS forums (A more general overview of all officer communities) I found a great link and a check list.

Under "Active Programs" click the OCS link at the bottom and it will load a Word document that has a checklist for an OCS Application. Should be very helpful to you, better than the other link I provided.
