It seems to me that doctrine wise, the days of “hot LZ” operations died when we exited SEA and Vietnam. It took Grenada and Somolia to remind us again and I don’t think there is a field grade officer or above out there that that would knowingly bring any rotorcraft into a hostile fire area, with few exception. Defensive suppressive fire just doesn’t seem to be a solution.
So now what?

The Sierra? Or maybe I'm misunderstanding your post.
I think the misunderstanding is when the helicopter is used as a troop insertion ship. I’m not a Sierra expert, but I imagine getting past that gun mount would be difficult when you only have seconds on an LZ.

Teams were always pretty quick to get in/out of the HH with a single door and the GAU17 installed. We were a little slower than our MH60L partners but not by much. Pave Lows didn't have much of an issue loading up the ramp with a .50 cal there either.

inserting teams is a different issue than grunts. The -22’s have a ramp gun like the Pave Low, but the article was asking for a forward firing gun to clear an LZ.

Out each door and the ramp? Not sure how big a V22 crew is - but that would mean 3 x CC/gunners - Pave Lows had that set up.
